Oxford interviews typically take place at the college you applied to. You will usually have two or three interviews of around 20-30 minutes each, sometimes at different colleges if you are pooled. The atmosphere is meant to resemble a tutorial: the interviewer gives you a problem and watches how you reason through it.

Yes. The official Oxford course page says all candidates must take the TMUA, and Oxford applicants must book for the October sitting.
No for this target cycle. The Mathematical Institute prospectus says that from 2026 Oxford will use the TMUA, that the MAT will not take place, and that Mathematics and Computer Science family applicants will take TMUA instead.
The registry says two interviews of about 25 minutes. Current Oxford Mathematical Institute guidance is less fixed: interviews typically last 20-30 minutes, usually have two interviewers, and applicants may have more than one at a college.
